What's the Missing Opportunity Here?

I've recently started working for a chiropractor doing in-home assessments on people who have been in car accidents (for those of you that care, it's just an interview and some range of motion tests, that combined take 45-60 minutes).

I'm trying to figure out how I can ethically cash in on this more so than I already am. For those of you who know me, I'm a fitness professional (I offer personal training as one of my services), so if the person hasn't been messed up too badly, I'll offer personal training or exercise program design (I design an exercise program for the person to follow on his/her own). But for the person who has been badly messed up by the accident, I am not able to offer personal training, because dealing with those people is outside of my scope of practice.

So knowing that, I've been racking my brain trying to figure out how I can ethically cash in on this? In other words, "what would Jay Abraham do?" What's the missing opportunity/opportunities here?
